Please Note: This sheet does not use the BMI percentile formula. You can easily calculate a child's BMI percentile to age percentage by using our online BMI calculator in the Nutrition Tools section.
This is only a diagnostic tool and does not definitely indicate a child's weight status. This should be used along with other accepted diagnostic tools.
